Individuals receiving services through Face Addiction Now (FAN) have rights protected by state and federal law.

If you have a concern or wish to file a Recipient Rights complaint, you may download and complete the Recipient Rights Complaint Form below.

A Recipient Rights Advisor will review your submission and contact you directly for follow-up or assistance.
